Are you looking for a health insurance program that offers stability and multiple plan designs all through a single point of contact? Then take a moment to view the newly formed AIA Indiana Group Association Employee Health Insurance Benefit Plan.

Firms will have the opportunity to apply for coverage at any time during the year. We are excited about this offering across Indiana. It will provide a stable insurance program with multiple plan designs, a technology platform and compliance support to any firm with at least 2 full-time employees (an owner counts as an employee).

Who will run this benefit program?
Anthem – Medical MetLife ancillary (dental, vision, life, accident, critical illness)
· Fully insured association plan
· Anthem provides access to the largest network in the state and provides members with access to any Blue Cross Blue Shield Provider across the county
· Choose from 16 different medical plan designs, 6 dental plans, 2 visions plans, Life & AD&D insurance, accident and critical illness plans

Shepherd Insurance (point of contact)
· Insurance Agent to manage our Association plan
· All questions regarding enrollment in the new plan should be directed to Shepherd Insurance at the contacts listed below.

Capstone Administrators
· Plan Administrators
· Provide: billing, compliance dashboard, enrollment and eligibility maintenance through Benefitsolver and general plan support

Indiana Employers Trust (IET)
· Our association will be under the IET umbrella
· This arrangement provided AIA Indiana with an opportunity to roll into an established program of 900+ lives

CLICK HERE to view summary highlights of the plans being offered through this program. You have the ability to pick plans that best fit your Firm’s needs. All firms that wish to review whether or not this program is competitive compared to your current plans or as a new coverage offering if you do not have an employee benefit program must submit Anthem medical applications. These will be reviewed by underwriting to determine a rate that will apply to your firm. You are not obligated to accept this coverage by going through the underwriting process, however, this is the only way to obtain a rate for comparison. You have two ways to complete the underwriting process:

  1. Have all employees complete and sign the attached Anthem medical application. Submit copies of the applications to Shepherd Insurance for underwriting review.
  2. Complete Anthem applications through an on-line system called FormFire. If your firm has used FormFire in the past, the system will need to be reset to show a December 1, 2021 effective date. Please have all employees complete and sign Anthem Medical applications based on a December 1st effective date. A pdf of the Anthem applications can be downloaded and sent to Shepherd Insurance for underwriting review. If you have not used FormFire in the past, Shepherd Insurance can get your firm signed up for this service at no cost.

In addition to the medical applications, you will need to submit the following items for review. These items will allow Anthem and Shepherd Insurance to verify they have applications on all full-time employees and to show similar plans to those you offer today:
· Excel census with date of birth, gender and type of coverage
· Current and renewal rates, if available
· Schedule of current benefits
· Current Carrier
· Effective date
· SIC or NAICS code
You will receive an offer that outlines the rates and plan designs based on information disclosed in the medical applications and submission of the items listed above. Information on the next steps to move forward in the program will be included in that offer.
